Your Care Needs

Will I have a dedicated team member of staff to manage my care?

Yes. Although all of our team will be involved in your care, one of our care team staff will be specifically assigned to you as your key carer. They will get to know you and your loved ones from your first day with us. They will learn your tastes and preferences and spend time to understand your specific care needs and the details of your care plan.

Will I have a personal care plan?

Yes. Before you join us, we carry out a pre-admission assessment. This enables us to have input from numerous health professionals – such as a GP, dietician, physiotherapist and ward sister – to consider your specific needs and develop an appropriate care plan. You, your family and friends are invited to be present and involved at this stage.
We try to make our approach as holistic as possible, because sometimes it’s the small things that make the difference. The more information we can learn about you, the more person-centred our care can be.

This is an ongoing process – we have something new to learn about our residents every day, which feeds into each individual care plan.

Will I and my family be involved in how my care is delivered?

Yes. Our first priority is that you are able to lead a fulfilling and happy life, and we actively encourage you and your family to become involved in decisions about your care and the way your Home is run. Importantly, we maintain an open-hours visiting policy, which means relatives and friends are welcome to drop by and see you at any time.

What happens if my care needs change in the future due to age, illness or a change in circumstance?

We carry out regular care reviews and work closely with external professional bodies to ensure that you continually receive the best possible care – physically, emotionally and spiritually.

If your needs change, we will discuss the management of your care with you and your loved ones and put in place relevant help (for example physiotherapy, or extra medicinal or nutritional input) to ensure your continued comfort and support.

Are your team members trained to a national standard?

We operate a robust quality assurance system, and care is formally monitored by our management team on a weekly basis. All our staff members receive regular in-house training, ensuring they are fully up to date with their professional development. Training is also provided by NHS professionals, who are experts in their specialised areas, such as Dementia and palliative care.
Our Homes are committed to working towards the Gold Standards Framework, the UK’s leading and most respected best practice and quality improvement programme for care homes.

Nellsar Limited is registered with and regularly inspected by the Care Quality Commission (CQC). Reports for all Nellsar care facilities are available at www.cqc.org.uk

Can I keep my own doctor?

Should you wish to keep your own GP you are welcome to do so, but we would recommend you speak to them before you move to make sure they are happy to support your request. Please be assured all our Homes are regularly visited by local GPs, dentists and opticians.

Will I be able to see a chiropodist/dentist/optician if needed?

Yes. These professionals make regular visits to our Homes to ensure all your health needs are catered for.
