Rehabilitation

In some instances, due to complex medical conditions or following surgery, a loved one may benefit from a period of rehabilitation. Many people go directly home from hospital following surgery, a heart attack or a stroke. For some however, additional support from qualified Nurses, Carers and Physiotherapists is an important part of recovery.

Recovery goals

Establishing and achieving goals for rehabilitation is an on-going process. With direct support for you and your family in those early days, we can help provide the confidence to achieve the planned recovery goals and speed up the recovery process.

Each of our Homes is designed to ensure that our residents live safely, securely and in comfort. All areas are accessible and clear signage enables residents to explore their Care Home and remain as independent as possible.

Individual Care Plan

Our person-centred approach means that all our residents are treated as individuals. Individualised Care Plans are based on an assessment of a person’s specific needs, and regular reviews ensure that they are constantly updated to ensure needs are always fully met.

Our Physiotherapist and team members will ensure that the goals for rehabilitation are realistic and achievable, and can be continued in when returning home.

Our flagship centre, Princess Christian Care Home, has an exceptional full-time Chartered Physiotherapist as part of their team – Leo Mathew. Leo began as a visiting Therapist in 2010, becoming a full-time employee in 2013 in order to best serve residents and the local Woking community.

If your loved one requires Rehabilitation, contact your choice of Home to discuss their needs with one of our Team, or why not come and visit us to see our facilities first hand? We look forward to welcoming you.
